On request, we provide service coordination and home and community support services to people between the ages of 18 and 64 who, due to a traumatic brain injury (TBI), need nursing home level care (NHTD), as well as to people between the ages of 18 and 64 who have a physical disability or are 65 years of age or older.
Service Coordination NHTD Help in the creation and execution of an individual, person-centered service plan that will promote the participant's independence, community integration, health, and welfare.

Service Coordination will make referrals to all services, research and secure opportunities for socialization or participation in activities of interest. SC will encourage with assistance at any time the participants and their families to ensure that there are satisfied with the services and they are active as well as all needs of the participants are being met.
